Important Notes:

  -- Name:  This MUST be your LEGAL name EXACTLY as it shows on your passport or official identification.  A passport is suggested, but you can sail with an original or certified copy of your birth certificate and an official photo ID such as a driver's license. (Note: if your birth certificate says Bertha Jocelyn Smith and you hate that name so your driver's license says B. Jocelyn Smith, call us.)
  -- Phone: Please give us a number where we can reach you easily.  We will NOT give this number to anyone else.
  -- Email: We'll use your email to keep you updated on the cruise.  If you don't have an email address, please call us.
  -- Payment Methods: Initial deposit is $100 per person, due with reservation.  Second deposit is $250 per person, due August 15, 2012.  The balance can be paid at any time prior to November 1, 2012.
        -- Credit Cards are acceptable at no additional charge.  Checks, if you choose that option, should be made out to Passport Travel, and promptly mailed to 414 E. Locust St., Nocona, TX  76255, after submitting the form below. 
        -- Submit the form, and then call 940.825.4121 to give us your credit card number.  That way there is no chance of your credit card number being stolen over the internet.  We need the number, the expiration date and the three- or four-digit number code, and we will safeguard that info as though it was our personal card.
